Services offered
My practice is different from other clinicians in that I charge based on my time and my fees for different services are as follows:
Initial psychiatric evaluation (45 minutes for preliminary work including sending and reviewing questionnaires, reviewing records and laboratory test results, etc.) plus a 75-minute live meeting via video conferencing. The combined fee for the initial psychiatric evaluation is $1400.
Follow-up visits (30 minutes; fee $350).
As noted above, I charge based on my time. So, if additional time is needed for reviewing records, filling out forms, contacting other doctors, and so on, I have to charge for that time as well at the rate of $175 per 15 minutes.
Insurance and fees
I do not participate in ANY health insurance.
Payment must be made either in advance or at the time of service. The following are accepted:
Credit or Debit cards (Mastercard, Visa, American Express) and Health Savings Account cards (Mastercard and Visa only).
Please enter your credit card information into our electronic system (Luminello) yourself. If you have difficulty doing that, you can ask Dr. Mago to do it for you.
Through your Luminello account, you can generate a billing statement for all charges and payments made, which you can submit to your insurance for possible partial reimbursement. This billing statement will contain all the information that is needed including the date of the visit, procedure code, diagnosis, charges, payments made, Dr. Mago’s information, etc. Please contact your insurance to check on whether you can get partial reimbursement for payments made to Dr. Mago.
Initial Evaluation
When the appointment for an initial evaluation is made, you will be sent an invitation to create an account on Luminello.com, the electronic medical record that I use.
Step 1: You will choose a password to make an account on Luminello.com.
Step 2: Enter your Profile information (address, date of birth, phone numbers, emergency contact, etc) into Luminello.
Step 3: Enter your credit card information to pay for the initial evaluation appointment. I charge for the initial evaluation in order to book the time for you. The charge will be refunded if you cancel at least seven days prior to the appointment. After that, the time is set aside specifically for you, and the appointment cannot be changed or canceled.
Follow up appointments
The secure medical record system, Luminello, will send you a confirmation of the appointment and a reminder 48 hours in advance. If you do not get an email confirming your appointment, please let me know immediately so that I can make sure there is no error.
How the appointments work
My practice runs by appointment. The advantages of this are:
1. I don’t run late
2. My clients get the full benefit of their time with me.
Missed appointments
Regarding missing an appointment, please note that I reserve a particular time for a particular person. So, if the person doesn’t come, I have to charge the full fee for that time slot anyway. I hope you will understand.
Canceling an appointment
I reserve a particular time for a particular person, I request at least 48 hours notice for cancellations. Otherwise, I have to charge the full fee for that time slot. I hope you will understand.
Prior authorizations
You may think that obtaining prior authorization for a medication, if needed, from your health insurance requires just a quick phone call. Nothing could be further from the truth! As a result, we have to charge a small fee for working on obtaining prior authorization, which is typically $50 but sometimes more if the insurance requires multiple steps, an appeal, etc.
